Apply for indemnity

In this application we are asking you correctly indicate your personal data which are necessary for us to provide the service you have selected and to contact with you. Information on your personal data processing is available in the  AAS "CBL Life" Privacy protection policy.

Information about the accident

Please note: the insurance does not cover minor bruises or contusions if they are not related to a fracture or a sprain and do not require surgical treatment. The insurance also does not cover illnesses that were not caused by an accident.

Information about the insured person

If an accident has occurred

Attach the documents issued by the medical institution. They must confirm the incident and describe the injuries sustained, for example, bone fractures or other health issues.

If the insured person has died

the compensation is requested by the beneficiary indicated in the policy or by the heir. If you are the designated beneficiary, attach a copy of your passport or identity document. If you are an heir, additionally submit the certificate of inheritance.

The size of one uploaded document must not exceed 5 MB, and the total size of all documents must not exceed 30 MB. Allowed file formats: .pdf, .jpg, .png.
